Due to the way Tilt in Space works, the fixed rotation to the semi-rest position is at the same angle, so will not stretch or pull at the lower back.

This fixed rotation follows through to fully reclined, taking the body comfortably into the neutral or zero gravity position, whilst giving full support to the body.
